SMH Heart Failure Care Team

Led by Thomas Archer, MD, the Heart Failure team includes more than 50 cardiology specialists, as well as highly trained cardiovascular nurse specialists for inpatient care. The nurse specialists provide in-depth education tailored to each patient’s diagnosis and needs, and provide follow-up for recently discharged patients to ensure a smooth transition from inpatient hospital stays to the return home.

Heart Failure Center

The goal of Sarasota Memorial’s Heart Failure Center is to alleviate heart failure symptoms and improve patients' quality of life. Our comprehensive approach has been shown to reduce frequent ER visits and hospitalizations associated with heart failure. We welcome patients at any stage of this chronic disease, providing the tools and support necessary to effectively manage this condition. By working closely with each patient and their care team, we create a personalized treatment plan to ensure successful long-term management and a better quality of life.

Outpatient services include:

  • Direct access to specialty-trained clinical staff
  • Personalized heart failure management plan
  • Expert optimization of medical therapies
  • Education for patients and families on heart failure, management strategies, and diet
  • Conversations on device therapies, including the latest advancements
  • Device monitoring to indicate fluid levels
  • Assistance with programs for high-cost medications
  • Discussions on advanced heart failure therapies
  • Support in exploring goals of care
